Hi folks,
I had a problem occur with my Raspberry Pi 5 and I am unsure whether its a software or hardware issue.
My Raspberry Pi5 fan and usb wont power off after shutdown and fan wont stop if booting without OS.
Replication steps:
1) Take Rpi5 with only Power (standard Rpi5 power brick) cable
2) Ensure there is no SDcard, USB device or NVME drive with OS connected
3) Connect power cable to trigger boot
4) Observe fan (in my case Active cooler fan) runs at full strength and never stops
Alternate replication:
1) Take Rpi5 with Power (standard Rpi5 power brick) cable, display cable and disk with OS
2) Connect power cable to trigger boot, fan spins up at full speed momentarily and shuts down after OS boot sequence begins
3) Boot into OS
4) Trigger shutdown either via terminal or by pressing power button
5) Fan starts blowing full strength indefinitely, if USB device connected power remains as well

Attempted fixes:
-sudo -E rpi-eeprom-config --edit
I modified POWER_OFF_ON_HALT=0 to POWER_OFF_ON_HALT=1
-Totally reinstalling the OS
- Updated the bootloader
I did days of searching through forums and google and all I can find is that it might potentially be a hardware issue.
What I think is happening:
Idea 1: The Pi is never able to enter standby mode when shutting down
Idea 2: On boot the Pi is stuck at 5volts due to hardware failure
Idea 3: Potentially firmware screwup, I was messing around with some terminal commands (rpi-update)
Im totally out of ideas so any help would be greatly appreciated
